This one hour class will present an overview of the article “A risk-benefit assessment strategy to exclude cervical artery dissection in spinal manual-therapy: a comprehensive review. (Chaibi and Russell, in the journal Annals of Medicine, 2019).” The authors offer the opinion that there is NOT “strong evidence in the literature that manual therapy provokes CAD.”

Doctors of chiropractic, as a regular part of their practice, will see patients presenting with neck pain and headaches. Spinal manipulative therapy can be part of an effective regimen for reducing symptoms and disability. Some case studies reported in peer reviewed literature have presented cervical artery dissection (CAD) as an adverse event related to this form of care.

The class will describe the levels of scientific medical evidence, including where case studies exist within this hierarchy. There will be review of the pertinent anatomy of the cervical spine and its arteries. While CAD is rare in the general population, the article proposes to inform practitioners in ways to better understand, assess, and most importantly exclude CAD from the differential diagnosis. The authors’ assessment algorithm, allowing providers to recognize potential red flags will be presented.

At the end of the class, the attendees will have a better understanding of the different levels and validity of medical evidence. They will improve their assessment skills for including CAD within the differential, and they will develop tools for creating an appropriate care plan.
